Upload the Zip, Or install from the WordPress Admin Area
Go to Appearance > GNT: Horizontal Subscribe
Configure settings and save
Check to make sure everything is working!

This widget creates a horizontal subscription form below the header for the Get Noticed Theme, as stated. Installation and setup were straightforward. It would be nice to have some more functionality in terms of controlling how the subscription form appears.
